Access to Medical Records Online

As from 1st April 2016 you are now able to request access to elements of your medical records online.

In order to access online services you must first complete an application form to request a Logon and Password from Reception. You will be required to show a proof of photo ID such as Driving Licence or Passport.

You cannot send someone else with your registration form and ID - you must come yourself.

Please ask at Reception for an application request for online services which will also inform you of the rules and regulations to accessing your records online.
